The guys in charge of it where definitely well educated in its set up and use and would often refer to their literature to make sure they were within specification. I think that somebody might be trying to cut some corners to save a buck or two, at the expense of safety, with these failures. I would think that with all the money that is in the entertainment industry, especially with a band like Radiohead, that there would be no expense spared to insure safety.
