I have never seen wall units in a kitchen to be installed above the window, have you?

Agreed, you need ladder, or something similar to this.

You can store the things that you use once a year on top shelves. 

Yes, in most cases the doors and units are custom made in this type of kitchens.

Here they have used the tiny space above the window for extractor hood.

Does anyone have any ideas about how to start with kitchen design? I've finished our bathroom which I designed myself and am pleased with with built in cabinets like a mini kitchen, but the kitchen is a bit more involved. It's quite a fairly small room and it has three doorways so there isn't a huge amount of space, so I think the layout needs some careful thought. Or perhaps the current layout is about as good as it can be?

You need to show the existing layout.
